Prerequisite Verification Form and FAQs

The Prerequisite Verification Form is used by OSHA to determine if you are seeking to obtain a trainer authorization to conduct Outreach training and other activities and if you have a minimum of five years of experience overseeing safety in the industry to which you are applying.

Frequently Asked Questions

Can the form be submitted in person?

No, all forms must be submitted via the OSHA Dropbox.

How far in advance can I submit the Prerequisite Verification form for approval?

The form should only be submitted for the most recent upcoming course. If you need to change your course date, you must resubmit a new PV form.

How do I know if I’ve been approved?

If verification documentation is approved, you will receive an email from CSUDH OSHA staff confirming the completion of your course registration (please allow three business days for processing).

Can I use the same Prerequisite Verification for other courses?

No; a form must be completed for each individual authorization.

Can I submit my resume in lieu of the prerequisite verification?

No; if additional space is needed, the individual can add this information on a separate document specifying the questions it’s addressing.

Do I need to submit proof of completion for the required prerequisite course?

Yes; the Prerequisite Verification Form should include a copy of the certificate of completion and/or official transcript for the required prerequisite course (i.e. OSHA 510, OSHA 511).

How long does it take for my Prerequisite Verification Form to be approved?

The verification process takes a minimum of three business days.

Will the Prerequisite Verification Form be processed sooner if I drop it off?

No; in fact, this may delay the process.

 


The Prerequisite Verification Form should include the following information:

  • Clear dates indicating you have 5 years of experience overseeing safety in the industry you are applying for (i.e. construction, general industry).
  • Each job title should list all task conducted to carryout the responsibility of overseeing the safety of others in the workplace.
  • Task should be written in complete sentences, with action verbs indicating you conducted the task.
  • Percentage of time spent on these responsibilities should be accurate.

Things that may not be relevant when completing the Prerequisite Verification Form:

  • Names of projects
  • Cost of projects
  • Number of employees
  • Public safety activities
  • Experience from other industries

Other Things to Consider

It is the responsibility of the applicant to ensure all course prerequisites have been met prior to submitting the Prerequisite Verification Form.

Please submit the completed Prerequisite Verification Form and all necessary documentation to the OSHA Dropbox at least 10 business days prior to the scheduled start date (unless you are resubmitting the form). 

Applicants should ensure the application has all the required information, including but not limited to:

  • Signature
  • Attached Certificate of Completion for prerequisite course
  • Course Dates
  • Complete all sections (including Section 41)
